TIL Cells

TIL Cells – Tumor-Infiltrating Lymphocytes in Cancer Immunotherapy

Tumor-Infiltrating Lymphocytes (TIL cells) are a form of personalized cancer immunotherapy that involves isolating immune cells from a patient’s own tumor, activating and expanding them in the lab, and reinfusing them to target and destroy cancer cells. TIL therapy has shown remarkable promise, especially in treating solid tumors like melanoma, cervical cancer, and lung cancer.

At RCERT (Regional Cell Research & Therapy Centre), TIL cell therapy is one of the next-generation immune strategies being developed as part of our commitment to leading cancer treatment innovation in Malaysia and Southeast Asia.

🔬 What Are TIL Cells?

Tumor-Infiltrating Lymphocytes (TILs) are a subtype of T cells that naturally migrate into tumor tissue. These lymphocytes are the body’s response to the presence of cancer cells. However, in many cancers, the immune environment within the tumor suppresses or “exhausts” these TILs, preventing them from functioning effectively.

TIL therapy extracts these cells directly from the tumor, reactivates them in the lab, and expands them to high numbers before reinfusing them back into the patient. Once reintroduced, these T cells are better equipped to recognize and attack the tumor.

🧪 How TIL Cell Therapy Works

The TIL therapy process is highly personalized and follows several steps:

  1. Tumor Resection or Biopsy: A small portion of the patient’s tumor is surgically removed.

  2. TIL Isolation: Lymphocytes that have naturally infiltrated the tumor are isolated from the tissue.

  3. Ex Vivo Expansion: The cells are cultured with interleukin-2 (IL-2) and other cytokines to multiply them by the billions in a sterile lab setting.

  4. Quality Testing: The expanded TILs are tested for potency, viability, and sterility.

  5. Patient Conditioning (optional): Some patients undergo lymphodepleting chemotherapy to prepare the immune environment.

  6. TIL Infusion: The TILs are infused back into the patient.

  7. Post-Treatment Monitoring: Patient immune response, tumor markers, and side effects are closely monitored.

The result is a highly targeted immune attack on the cancer — using the patient’s own cells.

✅ Benefits of TIL Therapy

  • Highly Personalized: Derived from the patient’s own tumor, making the response specific.

  • Broad Recognition: TILs can target multiple tumor antigens, even those unknown or unique.

  • Strong Cytotoxicity: TILs are activated and expanded to attack tumor cells aggressively.

  • Immunological Memory: Can help prevent recurrence and metastasis.

  • Effective in Solid Tumors: Especially beneficial in cancers like melanoma, cervical, lung, and head & neck cancers.

🧠 What Makes TIL Different from Other Cell Therapies?

FeatureTIL CellsCAR-T CellsCIK Cells
Antigen SpecificityPolyclonal (targets many antigens)Monoclonal (targets one antigen)Broad, non-specific
MHC RestrictionYesYesNo

TIL therapy is particularly valuable when the tumor presents many unknown or patient-specific antigens, which CAR-T may not detect.

🧬 Cancers Treated with TIL Therapy

TIL therapy has shown success in treating various solid tumors, especially:

  • Melanoma (most studied with FDA breakthrough designation)

  • Cervical Cancer

  • Lung Cancer

  • Head & Neck Cancer

  • Nasopharyngeal Carcinoma (NPC)

  • Bladder and Colorectal Cancer

  • Breast and Ovarian Cancer

At RCERT, we focus on regional tumor types, including nasopharyngeal carcinoma, liver cancer, and cervical cancer — all of which have strong potential for TIL application.

🏥 TIL Therapy at RCERT

RCERT integrates TIL therapy into its personalized cell therapy platform, which includes:

  • GMP-certified Cell Processing Laboratory (CPL)

  • Clinical collaboration with UNIMAS and Zhongrong Cell Technology Center (China)

  • State-of-the-art Cell Therapy Clinical Centre (CTC) in Sarawak

We are currently developing TIL therapy protocols tailored for:

  • Patients with advanced or metastatic solid tumors

  • Cases with resistance to chemotherapy or targeted therapy

  • Immunotherapy combination approaches (e.g., DC-TIL or checkpoint inhibitor + TIL)

⚖️ Safety and Side Effects

TIL therapy is generally safe and well tolerated, especially with medical supervision. Side effects may include:

  • Fatigue

  • Fever or flu-like symptoms

  • Temporary low blood counts (if preconditioning is used)

  • Mild inflammatory reactions

  • Rare cytokine-related side effects (usually from IL-2 support)

Compared to chemotherapy and radiation, TIL therapy offers a lower risk of long-term damage to healthy tissues.

👥 Who Is Eligible for TIL Cell Therapy?

TIL therapy may be suitable for:

  • Patients with metastatic or treatment-resistant solid tumors

  • Individuals who have failed conventional therapies

  • Candidates for clinical trials and research-based therapy

  • Patients with high TIL density tumors (as observed in pathology)

RCERT performs comprehensive pre-treatment evaluations to determine eligibility, including tumor biopsy, immune profiling, and medical screening.

🔄 TIL + Other Immunotherapies

TIL therapy can be enhanced when combined with:

  • Checkpoint inhibitors (e.g., anti-PD-1, anti-CTLA-4)

  • Dendritic Cell (DC) therapy to improve antigen presentation

  • Exosome therapy to stimulate TIL activation and persistence

  • CAR-T or DC-CTL in integrated regimens for refractory tumors

RCERT is actively exploring these multi-modal cell therapy combinations to deliver optimal results.

🌍 The Future of TIL Therapy in Malaysia

As personalized medicine and immunotherapy evolve, TIL cell therapy is poised to be a major contributor to the next generation of cancer treatments in Asia.

At RCERT, we are leading this transformation by:

  • Developing local TIL cell expansion and lab protocols

  • Partnering with regional hospitals for tumor sample collection

  • Launching clinical trials and compassionate use programs

  • Training medical professionals on tumor immune profiling

We aim to make TIL therapy more accessible, affordable, and applicable to Malaysian and Southeast Asian populations.

📝 Conclusion

TIL Cells (Tumor-Infiltrating Lymphocytes) represent one of the most personalized, tumor-specific, and promising forms of cancer immunotherapy. By empowering the patient’s own immune cells to fight cancer, TIL therapy offers hope for those facing aggressive or resistant solid tumors.

At RCERT, we are committed to delivering cutting-edge TIL therapy through clinical excellence, scientific innovation, and personalized care.
